Below is a link to the pre-operative screening questionnaire that the nurses will be addressing with you by phone. The nurse will also ask questions in regard to your medical history and questions that are needed to keep you safe for your whole surgical day.
You may print out the page and fill in the areas during the phone conversation so it may be easier for you to discuss any information that would assist in making the process easier.
