We are committed to providing a website that is accessible to the widest possible audience. Request Medical Records

If you would like a copy of your medical records after you're discharged, please allow 24 to 48 hours for record completion.

To request to have your medical records sent to a physician, please fill out this form and fax it to 561-488-8338.

To obtain a copy of your records, please fill out the form below; however, you must come in person to the Health Information Department to retrieve the copies. There is a minimal fee for your records.
